TERMS, CONDITIONS, and RELEASE FORM to PRINT & FAX: 213-291-1473 A. Terms and Conditions: The following terms and conditions relate to the auditioning of a minor child WHOSE NAME IS, _____________________________ (please print) and who is 5, 6, 7, 8, 9, or 10 (please circle one) years-old. I understand that the selection of the 150 children will be grouped by race, and that those groups have been allocated a maximum number of children based on the needs of the production as outlined by the Producers. My child will be registering to compete in the Hispanic, Arabic , Native American, Black American, Asian, Hawaiian or other Pacific Islanders, or Caucasion group slot (please circle one only). I am the parent or legal guardian of the above listed child and I do hereby give my permission for this child to participate in the “Lundon’s Bridge and the Three Keys” motion picture auditions that has been introduced via the website known as: www.Lundons-bridge.com. I, the undersigned, do understand that by placing our child’s audition tape on the Internet via the You Tube site, that all voting that may be done by the public via www.lundons-bridge.com or by any other means adopted for voting by producers, that if our child is not selected to move up to any of the next levels, that there will be absolutely no recourse afforded to us and that there will not be any recount of the votes authorized. It is understood that the voting for this audition will be done via the official Lundon’s Bridge website by the public or by telephonic voting over the telephone by the public. Either method is not considered a verifiable method of voting, but all participating parties, by virtue of signing this document, accept this method as the voting method for this audition effort. If there are even or equal counts (ties) in any of the race or gender categories, the producers will have the irrevocable right to select a winner based on performance and that this selection or choice by the producers will be final and irrevocable. If the producers end up making a selection, their decision will override the vote count. We understand that based on race and gender; there will be a numbered quota for each group accepted for a position in this movie. The purpose of this restriction is to maintain a sense of fairness in the number of children from any one race. There are only 151 roles being sought in this audition. One of the roles is the lead role of an 8 year-old half white/half Hispanic female. This role will not require the actress to actually be half white/half Hispanic but rather a ‘look-a-like’ of those mixes. The character could be a child with any shade of blond hair, black hair, brown hair, or red hair, curly or straight, long or short. The key is that the child look like she could be of the mentioned mix race and age. Of the 150 remaining roles, they will be a mix of featured and special effects roles. That means some of the children will be required to do acting in a special effects setting wherein they will be turned into cute, flying bugs, digitally of course. Some will have speaking lines and others will have only wild lines, which means not scripted words, but asked to adlib lines as they play with the other children during the scene. Though it will not be exact, we will try to keep the gender numbers as even as possible--seventy-five boys and the same number of girls. Out of these numbers the producers have slotted 30 roles as African American (15 boys/15 girls). Equally, from the Hispanic group, there will be 30 children (15 boys/15 girls). We will give another 12 children (6 boys/6 girls) from the Asian population. American Indians will have 6 slots (3 boys/3 girls). Our Arabic children will also be slotted for 6 kids (3 boys/3 girls). The Hawaiian or other Pacific Islanders group will also get 6 slots for the kids (3 boys/3 girls), and Whites will have the remaining number of position for children in those ages which is 60 (30 boys/30 girls). If there are not sufficient children auditioning to fill the quotas, the producers will have the right to fill those slots through this campaign via the normal casting process. B. Release Form: The undersigned, being the legal parents(s) or guardian(s) of ______________________ a minor child (hereinafter referred to as the “the Child”), being of legal age and residents of the state of ________________, in the city of ________________, do hereby consent and grant to C-It Entertainment Group, LLC (hereinafter referred to as the “Producer”), permission (1) to grant full use of a digitally recorded audition tape of “the Child” performing a monologue scene that will be used for the purpose of soliciting public votes that may win “the Child” a position as a cast member in the to be filmed, feature motion picture entitled, “Lundon’s Bridge and the Three Keys.” This video is not intended for any other use with the single exception that the video may be used as a possible audition sample (demo) shown only on www.lundons-bridge.com or www.youtube.com and if this is used as a sample audition, the permission given here also covers this use. The same permission applies to any still photo sent into C-It Entertainment Group, LLC of “the Child” for the purpose of placing a link to view the “Child’s” video on YouTube. In addition, permission is also granted to “Producer” to allow the video to be shown on network television, cable television or public television for the purpose of advertising and marketing the campaign either wholly or partially including talk shows and for the final use as a television show that will help provide a stage or platform to obtain votes for “the Child’s” performance, so long as such use is in connection with the exhibition, advertising, promotion, distribution and/or exploitation of the audition-based show. The undersigned further agree and warrant that the above-mentioned minor child, will not disaffirm or disavow this consent and permission on the grounds that “the Child” was a minor on the date of execution thereof or any similar grounds whatsoever, or endeavor to recover from “Producer” or any of its individual shareholders, directors, officers, employees or other associated persons, or through any guardian, any sums for participating in the audition campaign, other than those sums specified in the future Child Actor Employment Agreement in the case that “the Child” does win a part in this to be filmed feature motion picture. In conclusion, by my signing below, I confirm herein that I have read the above terms and conditions as well as the release statements, and agree to each of them as written.
